Therapy Programs


Our senior care program is a comprehensive rehabilitation and fitness program specifically designed to improve the functional independence of the elderly. The program is a first-line of treatment for a wide variety of conditions including edema, soreness, stiffness and general weakness secondary to acute or chronic orthopedic and/or neurological disorders and post-op debility.

The program combines evaluation, education and rehabilitation treatments including therapeutic activities, exercise and modalities including electrical stimulation and heat/cold therapy, all focused to promote functional independence.  Once therapy treatment goals are met and prior to discharge, clients are prepared to continue their specially-designed fitness program, enabling them to continue to build upon their initial gains.

Fall prevention

Falls are the second leading cause of accidental death in the United States and 75% of these falls occur in the senior population.  Falls not only affect the client’s quality of life but can have a significant psychological impact.

Our clinicians work directly with the client to develop a customized fall prevention program that may include safety equipment installation, improved lighting, removal of ambulation obstacles, coordination with Physical Therapy for appropriate use of assistive devices and mobility exercises for pain reduction, and tele-monitoring device. The objective of our fall prevention program is to educate clients about the causes of falls and proper preventative measures, ultimately creating an environment that is safe for independent ambulation.

Hand/Upper extremity injury Assessment and Rehabilitation

Our client-focused comprehensive approach to Occupational Therapy evaluations include not only musculoskeletal (e.g., muscle strength, range of motion), sensory, cognitive or perceptual, vascular, skin, or connective tissue assessment, but also pertinent past medical and vocational/avocational history.  Clinicians also identify psychosocial, environmental and other factors that may influence rehabilitation outcomes.

A comprehensive plan of care for hand and upper extremity injury may include therapeutic activity and exercise, Orthotic/Adaptive/Assistive device selection, design, fitting, fabrication and product training,  joint protection and/or energy conservation techniques, desensitization, ergonomic and activity modification in home or leisure, and ADL/IADL training.

Joint Replacement/Post-Op Program

Physical Therapy is the most important factor in regaining strength and mobility post-surgery.  Our total hip and knee replacement rehabilitation programs follow post-surgical rehabilitation protocols focusing on individualized treatment interventions.  The programs involve a variety of treatment options with the goal of restoring the client’s strength, function and mobility for resuming normal activities of daily living.


Neurological Rehabilitation

Our team of skilled professionals provides a comprehensive, multi-disciplinary approach to neurological rehabilitation that includes muscle and sensory re-education, coordination activities, range of motion exercises and speech therapy to patients who have experienced a neurological disorder such as stroke, spinal cord injury and Parkinson’s disease. The goal of rehabilitation is to enable a client who has neurological deficits to reach the highest possible level of independence and regain optimal quality of life.

Oral motor weakness rehabilitation program

Speech-Language Pathologists treat adults with varying degrees of  language, cognitive, speech and feeding difficulties resulting from stroke or progressive neurological conditions such as Alzheimer’s, Dementia and Parkinson’s disease.

Our team of clinicians develops and implements a specific Oral Motor Therapy program designed to increase coordination and strength of the jaw, lips, tongue and accessory muscles. Treatment also includes exercises to improve feeding and minimize swallowing difficulties. Our therapists also counsel and support clients and their families on the nature of specific disorders and teach proper compensatory techniques, instilling confidence and enabling them to better independently manage symptoms and improve quality of life.

Orthopedic Rehabilitation Program

Orthopedic rehabilitation primarily focuses on the problems of the musculoskeletal system.  It is essential for the client to regain their prior level of function, strength and motion right after injury or surgery.  Orthopedic clinicians diagnose, manage and treat injuries of the muscles, bones ligaments and tendons and rehabilitate patients to their optimal function.

Orthopedic patients typically have deficiencies and weaknesses, which could be treated with specific targeted workouts.  Our specialized clinicians design and implement a target workout to restore and enhance function and minimize presented problems.  Our orthopedic clinicians are educated in surgical procedures and are able to adapt and adjust their knowledge to treat each individual patient.

Orthopedic rehabilitation programs and treatments include but are not limited to muscle stretching and strengthening, balance/proprioception, modalities to decrease swelling and increase circulation and accelerate healing times.

Apex Home Health Care, Inc. clinicians can provide continuing close holistic and supportive care of the cancer client at home, particularly assessments of their systemic changes that need to be reported to the physician, promotion of their palliative comfort as necessary, and assisting them in the management of their schedule for chemotherapy, radiation therapy and other cancer treatments.

Cardiovascular Disease

Our clinicians provide the highest quality care via thorough assessment and observation and directly work with the client’s physician to develop a customized plan of care. Our cardiovascular program is geared towards addressing common diseases including CVA, Heart Failure, Hypertension, Hypercholesterolemia and Peripheral Vascular Disease. Apex HHC clinicians achieve effective outcomes by educating the client/family/caregiver on proper medication management, lifestyle modification (stress reduction, dietary modification, special equipment), and care coordination with physical/occupational/speech therapy as needed.

Cognitive Impairment (Alzheimer’s Disease/Dementia)

Cognitive Impairment is one of the most common diseases affecting the elderly and their ability to safely function independently. As the disease progresses, behavioral symptoms increase, hence the need for a better understanding of the disease process. Apex HHC provides a holistic approach in ensuring that the caregiver/family will be able to provide a safe and structured environment. An Apex HHC clinician, who is specially trained in the field of behavioral management, will thoroughly evaluate each client’s specific needs and implement a comprehensive treatment regimen focused on achieving an effective response to medication as prescribed by the client’s physician.

Diabetes and Insulin Management

Diabetes is a complicated disease that if left unmanaged can affect the body in its entirety. Here at Apex Home Health Care, Inc., our clinicians will work directly with newly-diagnosed and longstanding clients to educate them on Type I or II disease to successfully control their blood glucose levels not just by medication but through non-pharmacologic means such as proper diabetic diet, ideal body weight maintenance, physical exercise and regular physician follow-up. Our Diabetes program also focuses on balancing all the factors including stress reduction strategies that can contribute to the promotion and maintenance of acceptable blood sugar levels.


ESRD/Renal Failure

Apex Home Health Care, Inc. clinicians work with dialysis clinics and other healthcare personnel to periodically review the client’s overall status. Our skilled clinicians will provide a holistic approach to the renal client’s care through education and support to help them cope with their disease, manage symptoms, and improve quality of life.

Genitourinary Dysfunction

Genitourinary Dysfunction causes anxiety and decreases self-confidence. Our goal is to uplift dignity and confidence of our clients. Our GU clinical program involves imparting knowledge and training of our client/family/caregiver in proper clinical and psychosocial management of common issues including: Incontinence, Impotence, Urinary Tract Infections, and Benign Prostatic Hypertrophy. Specific modalities include timed-voiding training, medication compliance, and proper perineal hygiene and infection control measures. The goal of this program is to not only to address clinical issues, but also to promote and maintain the client’s dignity, independence and self-confidence.

Home Safety/Fall Prevention

Most hospitalizations and readmissions are due to fall. With an assembly of skilled clinicians, we provide an evaluation of potential environmental contributing factors that may increase risk of at-home incidents, a conditioning program focusing on strengthening of core muscles, and educating family/caregiver on home safety and proper use of assistive devices. Our skilled staff provides a thorough evaluation of clients’ functional limitations and capabilities and will coordinate with other services including physical/occupational therapy and medical equipment provision if necessary.

Nutrition and Hydration

Every client admitted under our care undergoes a comprehensive physiologic assessment by our skilled clinicians, which includes Nutritional and Hydration profile. Malnutrition and dehydration can greatly affect a person’s quality of life and are often associated with co-morbid conditions. Apex HHC clinicians educate the client on the importance of achieving their optimal weight through exercise and by following a dietician-prescribed dietary regimen.

Pain Management

Chronic pain is one of the most common and debilitating conditions seniors face in a home health setting. Our multi-disciplinary approach to management is to develop a customized treatment program that not only addresses pain symptoms, but the whole person. Apex Home Health Care’s goals are to reduce the subjective experience of pain, assist with medication management, and improve the individual’s ability to control pain, physical and emotional functioning and thereby fostering a meaningful quality of life.


Respiratory (COPD/Asthma/Tracheostomy)

Evaluating level of physical limitations and capabilities is the primary guiding tool in taking care of clients with a respiratory condition. COPD & Asthma are the most common chronic diseases that exacerbate due to environmental exposure, thus proper education on compliance with prescribed therapeutic measures can greatly improve respiratory function. Apex HHC clinicians are also trained in specialty respiratory care programs including tracheostomy care, pulmonary toilet and PleurX drainage. Our respiratory program focuses on empowering clients with clinical knowledge on the nature of their disease, avoiding precipitating factors (i.e., allergens, smoke), signs/symptoms of exacerbation, importance of compliance with treatment regimen and emergency care measures to ultimately improve quality of life.

Specialty Programs (Parenteral/Enteral Treatment, Infusion and Therapeutic Drug Monitoring)

Apex clinicians provide exceptional service in managing clients with special needs such as medication infusion, intravenous hydration, specialized nutrition, and therapeutic drug monitoring. Our nurses are highly trained to integrate aseptic techniques in collecting specimens for evaluation and if needed, a skilled phlebotomist will be available. Apex works closely with client’s physician in ongoing treatment of the patient, with timely reporting of laboratory results and proper education of client/caregiver/family on changes in medication regimen.
